To request a consultation with Mr John Cunningham please fill in the below registration form. If you prefer we can mail you one or you can download it here. Before seeing John for treatment, you will need a referral from your doctor or other health professional. We need to receive your referral prior to making you an appointment. If you have received a registration form in the mail, please fill it in and bring it with you.
